Sharing Moments

The Care Team Connect app is designed to help caregivers share enjoyable moments with others on the team. Describing pleasant or uplifting moments can not only help provide peace of mind for loved ones, but it can also help inspire future engagement. 

WHAT IS A MOMENT? 

Memory loss symptoms can cause care recipients to feel reserved, worried, and/or frustrated. Moments are activities, topics, or events that help care recipients avoid these negative emotions and feel at ease and connected to cherished memories.  

Moments may include a wide variety of activities, such as: 

  • Singing along to a favorite song
  • Sharing a story
  • Laughing at a silly video
  • Going for a peaceful walk
  • Visiting with friends or family
  • Enjoying a special meal or dessert
  • Petting a friendly dog or cat

Caregivers can use the Care Team Connect app for engagement inspiration. As family members share photo, topics, and music, caregivers should explore opportunities to engage the care recipient accordingly.  

Caregivers should not view moments as tasks to be completed; instead, they should allow these moments to happen naturally.  

Moments should focus exclusively on positive emotions and experiences. If a care recipient is having a difficult time, caregivers should not document those challenges via a moment. Instead, they should follow existing procedures for dealing with challenging behaviors.  


Recording a Moment

The following steps show how to add a moment when logged into the Care Team Connect app as a caregiver.

1. Add Button To add a moment, tap the Plus button.

2. Add a Moment Select the option for Add Moment.

3. Choose Activity Select the activity that the care recipient enjoyed. 

Then tap Next button.  

4. Get Specific After you select an activity, you will be prompted to select the specific photos, topics, or music selections. 

Make the necessary selection(s), then tap the Next button. 

5. Choose a Mood Outcome Select the mood the best describes the moment. 

Then, tap the Next button. 

6. Describe the Moment In sentence or two, share why you thought the moment was meaningful. 

When finished, tap the Done button.

The goal of reminiscing is to help people with dementia feel valued and connected to happy memories in their past. Using a tablet can help caregivers facilitate reminiscing activities; as they can, explore images and videos connect to the recipient’s past experiences.
